ILLNESS CERTIFICATES REQUEST
This form is to enable patients to request 'Illness Certificate' digitally. Alternatively it can be posted as usual to Summerhill Primary Care Centre, Summerhill, Co. Meath, A83 V252, Ireland.
If you're unable to work due to illness, you may be entitled to Illness Benefit from the Department of Employment Affairs and Social Protection (DEASP). To apply, you need a medical document called a ‘Certificate of Incapacity for Work’ from your GP. Your GP will complete and may submit this certificate electronically through a secure system. In some cases, one certificate may cover your entire period of illness, but if your condition continues, you may need to return for a follow-up. Your employer might also ask for a separate medical certificate (often referred to as a 'sick note' or 'work cert') during your sick leave. This is commonly required if you're absent for more than 2 or 3 days in a row. Please note: you must see a GP to receive either an Illness Benefit certificate or a medical certificate. If you need to extend either one, you can submit the form below. We’ll contact you if another GP review is necessary. All submissions will be dealt with by a receptionist in the next 7 days.
